Most of our forms are available in several formats. If there is a format you prefer, and we can provide it, please let us know. We are also happy to assist you with questions and will try out best to help you in utilizing our documents.
Microsoft Word Forms
Please note that you must have MS Word or the MS Word Reader (free-see below) installed on your computer to use the fill-in forms feature. Most forms were set up using MS Word 2003/Windows so these may not work with earlier versions.
The fill-in forms use a feature of Microsoft Word that makes it easy to complete your form or report on your computer. Download the fill-in form document and fill in the form using Microsoft Word. Enter information in the “fields” of the form (these may show as grey boxes; don't worry, the shading doesn't print). To move from field to field, use your mouse, the arrow keys or press Tab. Use your mouse or the spacebar to mark and unmark a checkbox.
When you click on the link for the file, you will be asked whether you want to "Open it" or "Save it to disk".
If you select "Open it"
The application will launch in Microsoft Word, and bring you to that file. You
can work in the file, using Tab to get to each form field. At this point you can
either print the file, or save the file on your computer.
If you choose "Save it to disk"
You will prompted to indicate where the file will be saved on your local or network
hard drive. Once the file is saved, you have a blank file to enter data in. Open
it in Word. You can enter data into the form using Tab to get to each form field.
Remember to rename the filled-in file before you save it or the blank form will
be over-written.
PLEASE REMEMBER TO SAVE YOUR MODIFIED FILE WITH A NEW NAME. You may now either print or email the completed form.
